A complaint must be filed within 180 days to the date of the alleged discrimination. The complaint should include:

1.  Name, address and telephone number of complainants
2. Name and address of the agency, institution, or department complainant believes discriminated against him/her
3. Include as much specific, detailed information as possible about the alleged act of discrimination, and other relevant information
4. Name of any person, if know, who can be contracted for clarity of allegations
5. Complaint must be signed
